Liability Waiver

By submitting this application, I understand that Operation Pumpkin and the City of Hamilton and their agents, employees and officers will not be held responsible for any damages, injury, or illness that may occur as a result of my participation in the Operation Pumpkin Pet Parade and Costume Contest. Further, I understand that there are dangers inherent to such an activity and I agree to assume all responsibilities for my actions and the actions of my pet(s). I also affirm that the pet(s) I will be participating with has current rabies vaccination (if applicable). Further, I acknowledge that Operation Pumpkin, its agents, employees and officers reserve the right to deny participation to any entry (human, pet or both) on the basis of public safety. Having read this waiver and knowing the facts, and in consideration of the acceptance of my entry, I hereby for myself, or anyone else who might claim on my behalf, covenant not to sue, and waive, release and discharge Operation Pumpkin, the City of Hamilton and its agents, employees and officers. I also understand that Operation Pumpkin and/or any sponsor may use, for publicity or promotional purposes, my name, picture or video or my pet without liability or obligation to me.
